If you would like to register at the surgery and are living within our boundary area, please come to the reception desk and ask for the registration forms. The forms ask for your name, address, NHS number (if you know it) and other information, such as, the name and address of your previous GP.
We will ask you for proof of identity, for example, a photocard driving licence or a passport and proof of your address such as a utility bill (gas, electricity, water or telephone bill but not a mobile telephone bill) or a council tax bill.
We also ask you to complete a health questionnaire and from that may ask you to come to the surgery for an appointment with the nurse. This will be to make sure that any tests or checks that you need are up to date.
Online Services
We offer our patients online appointment booking, repeat prescription requests and online records viewing through a system called Patient Access, (click here to read more). You can ask to register for this service when you come to join the practice.
Change of Personal Records
If you move house, change your name or telephone number please remember to let us know so that we can contact you, if necessary. You may telephone 01295 758372, fax 01295 750435, email Cropredy.surgery@nhs.net or call in person to let us have your new details.
